The Medicare-covered home evaluation is an assessment only. It does not mandate immediate modifications or equipment installations. Instead, it documents the patient’s current home environment and identifies potential barriers or risks.

For example, if a patient has seven steps to enter their home, that detail becomes part of their medical profile. If they later experience a leg injury, hip surgery, or mobility decline, their doctor already has documented evidence to guide discharge planning, home health services, and safety recommendations.
Importantly, this evaluation must be initiated by a physician. While patients and families can prepare and ask questions, the assessment itself is a Medicare billable service ordered through the doctor.
